Medicare does not currently require or prohibit split billing of diagnostic tests. There have been two transmittals published by CMS that would require split billing of these services in situations where the various components of the tests are rendered on different dates of service: Transmittal 1823 and Transmittal 1873. Both of these transmittals have been rescinded and no more recent CMS guidance could be located.
The fact that Medicare has twice proposed to require split billing establishes that it is not inappropriate to do so. The fact that the transmittals have been rescinded illustrate that billing globally for the tests is not currently inappropriate
